Recommitment Forms
Each year our currently enrolled students need to state whether they intend to accept a seat for the next school year. Be sure to fill out the recommitment form as soon as possible. If you are currently a high school student or will be high school student this coming fall, please click here to retrieve a recommitment form.
Applications
UPSM Schools will be accepting applications for the 2012-2013 school year beginning January 23, 2012 through Monday, March 12, 2012. Don’t wait! Your child deserves a UPSM education.

Once your application is complete, you may:
- Drop it off at the front desk of the necessary school. Please drop off all middle school applications to the middle school and all high school applications to the high school:
- We do not recommend faxing applications, however, if you choose to fax or mail, please call the administrative office to verify receipt.
- Be sure to get your application dated and signed.
